The data in your care plan are visible in the Gezond app, in the Care Plans tab, and are also visible to the GP(s). After termination of the care plan, the data in the plan are archived, so that you can still access the care plan afterwards.
You can delete the data if necessary by deleting your account from the app. Deleting your account clears your entire history in the app and is no longer visible to you and the GP(s).
