How do I fill in the surveys of a care plan?

A care plan may offer one or more surveys for you to answer.

You can view the survey as follows:

Step 1. Open the Gezond app

Step 2. Click on the Care Plans tab

Step 3. Go to View all content

Step 4. Click on the Survey

Step 5. Click on Start Survey

Step 6. Answer the Questions
