How do I fill in the diary of a care plan?

A care plan may offer a diary consisting of a number of (fixed) questions that you can answer each day.   

You can view the diary as follows:  

Step 1. Open the Gezond app  

Step 2. Click on the Care plans tab  

Step 3. Go to Sleep diary  

Step 4. Click on Answer questions  

Step 5. Click Continue  

Step 6. Choose a Date  

Step 7. Click Next  

Step 8. Answer the Questions
