How do I provide feedback on a care plan?

You can often give feedback in the care plan at the weekly themes, or in the surveys. You can also send feedback on the care plan to email address help@gezond.nl or Submit a ticket. Please include the following:

Full name
Date of birth
Subject
Feedback

Never send privacy-sensitive (medical) information, such as your BSN (social security) number!
